Description
Mental Health Cooperative is a mental health agency that incorporates intensive case management, psychiatric/clinic services and 24 hour emergency psychiatric services into an integrated system of care. Our services assist children and adults who have a serious mental illness to live successful and satisfying lives in the community and recover from the devastating effects of the illness.
Services are delivered through a multi-disciplinary team consisting of case managers, a team supervisor, psychiatrist, advances practice nurse and nurse. Services are tailored to meet the unique needs of each individual by focusing on the achievement of personal goals while helping the individual gain control over his or her systems.
Mental Health Cooperative is currently recruiting nurses fto work in our Davidson and Montgomery County offices.
Schedule - Monday - Friday, 8am - 4:30 pm
Position Summary: Serve as liaison for consumers between the Agency medical staff, case managers and the medical community. This liaison consists of interpretive, instructional and hands-on care.
Position responsibilities:
1. Attend, participate and receive supervision in weekly multidisciplinary team meetings.
2. Works scheduled clinic with Team’s provider.
3. Interpret, process and implement provider’s orders.
4. Develop and maintain consumer records for the smooth implementation of medication orders.
5. Order prescribed medications for consumers from designated pharmacy for timely dispensation. Check InfoScriber for up-to-date orders.
6. Face-to-face with consumers in the community (home, restaurant, day treatment program, hospital, etc.) to provide continuity of care in the physical and/or psychological domain.
7. Conduct weekly Clozaril clinic (nursing assessment/blood draw and providing medication).
8. Serve as patient advocate.
9. Represent the Agency with visitors learning about the Agency model.
10. Coordinate all nursing functions for designated team(s).
11. Complete vital signs, weights and AIMS on consumers in a timely manner.
12. Check record for current Primary Care Physician(PCP) release of information.
13. Fill in for other site/satellite nurses on a PRN basis.
14. Nursing assessments completed for walk-ins; consult with team providers as needed.
15. Assist case managers as needed.
